Chapter 3 of Steve Blank's Four Steps to the Epiphany instructs you to State Your Hypotheses as Phase 1 of the Customer Discovery Process. In the process of doing that, I put together a template with the relevant outline of questions that need to be answered. Sharing the document with the hopes that it helps other lean entrepreneurs.
